Police: Person killed in SW Side crash

(CHICAGO) One person was killed in a crash early Thursday on the Southwest Side, authorities said.

A car collided with a garbage truck shortly after 4 a.m. on Harlem Avenue near 63rd Street, according to preliminary information from Chicago Police and Fire Media Affairs.

One person was dead at the scene, authorities said. Further details were not immediately available.

The Cook County medical examiner’s office has not released information about the death.

Fire officials: 1 dead after Metra train hits vehicle on NW Side

(CHICAGO) One person was killed when a Metra train on the Union Pacific-Northwest Line struck a vehicle Thursday morning in the Norwood Park neighborhood on the Northwest Side, fire officials said.

Emergency crews were responded about 7:10 a.m. to reports of a train striking a vehicle near Nagle and Northwest Highway, according to Chicago Fire Department spokesman Jeff Lyle. Details about the crash were not immediately known.

Inbound train No. 610, which was scheduled to arrive in Chicago at 7:22 a.m., struck the vehicle, according to a service alert from Metra.

One person was pronounced dead at the scene, Lyle said. The Cook County medical examiner’s office did not provide information on the fatality.

Trains were halted in both directions near the Gladstone Park station as a result of the crash and Metra was anticipating “extensive delays.”

UPS Sued Overly Deadly Chicago Crash

(CHICAGO) United Parcel Service is being sued by the relative of an elderly woman killed when she was struck by a truck, on Chicago’s northwest side.
Just before 5 p.m. on August 22nd, Maya Medovaya was  crossing Peterson Avenue at Central Park when she was struck by a UPS truck as it turned right onto Central Park.  The 78-year old woman died, at a hospital, 20 minutes later.
Medovaya’s daughter is suing the company, seeking more than $50,000 in damages.
The suit also lists the driver as a defendant.  64-year-old Jackie Clay was ticketed for failure to yield to a pedestrian in a crosswalk.
UPS says they are sad for the family’s loss and it’s aware of the lawsuit but won’t comment on active litigation.

Fire officials: 10 injured in 3-vehicle crash on Dan Ryan Expressway

(CHICAGO) Ten people were injured in a three-vehicle crash early Tuesday on the Dan Ryan Expressway, fire officials said.

The crash happened about 12:35 a.m. in the inbound lanes of I-90/94 near 95th Street, according to Illinois State Police. Information on the circumstances of the crash were not immediately available.

Seven people were taken to hospitals in serious-to-critical condition, while three more people were hospitalized in fair-to-serious condition, fire officials said. Their ages and genders were not known.

As of 2:45 a.m., all inbound lanes of the expressway were closed to traffic, police said.

Metra: Pedestrian fatally struck by train in Forest Glen

(CHICAGO) A pedestrian was fatally struck by a Metra train late Wednesday in the Northwest Side Forest Glen neighborhood, according to the transit agency.

The person was struck by an inbound Milwaukee District North train about 11:30 p.m. near the Forest Glen station, 5310 N. Forest Glen Ave., a Metra spokeswoman said.

The Cook County medical examiner’s office could not confirm the death early Thursday.

The inbound train was stopped for about 2 hours, according to Metra. The last outbound train departed 90 minutes late.

Oak Lawn man dies after Metra train strikes SUV

(OAK LAWN) Authorities say a suburban Chicago man has died after the SUV he was driving was struck by a Metra SouthWest Service train.

Metra spokesman Tom Miller says the collision occurred about 7:40 a.m. Monday after the SUV was driven around crossing gates at 98th Street and Central Avenue in Oak Lawn. He says all bells and bells and lights at the crossing were functioning.

The SUV driver was pronounced dead at a hospital. The Cook County Medical Examiner’s office identified him as 59-year-old Arthur Hornsby of Oak Lawn.

The crash delayed commuters on the line. Miller says all trains began moving again by 9 a.m.

Car, pickup crash into buildings in Belmont Gardens

(CHICAGO) The white Ford pickup was traveling westbound about 10:25 p.m. in West Fullerton Avenue when it struck the side of a Nissan sedan, according to Chicago Police.

The Nissan crashed into the Fullerton Animal Clinic, 4303 W. Fullerton, and the truck crashed into the Immigration Law Office, 4305 W. Fullerton, police said.

No injuries were reported, and police said alcohol may have contributed to the cause of the crash. As of early Monday, no citations or charges were filed.

(WESTERN SPRINGS) A person was killed when a semi truck crashed into a wall early Tuesday on the Tri-State Tollway in west suburban Western Springs, police said.

The semi was headed southbound on I-294 at 2:31 a.m. when it hit the median wall just north of 47th Street in Western Springs, according to Illinois State Police.

One person was killed in the crash, but further details were not immediately available, police said. The Cook County medical examiner’s office could not immediately confirm the fatality.

As of 5:45 a.m., the left lane on the southbound side of I-294 remained closed to traffic, according to police. The two left lanes remained closed on the northbound side.

Bicyclist struck by SUV, critically injured in Bridgeport

(CHICAGO) A man was critically injured Monday afternoon when he was struck by an SUV in the Bridgeport neighborhood on the South Side.

Joshua Thomas, 26, was driving a 2000 Dodge Durango in the 3500 block of South Lowe Avenue about 6 p.m. when he made a U-turn and struck a bicyclist, according to Chicago Police.

The bicyclist, a 47-year-old man, was taken in critical condition to Stroger Hospital, police said, adding that he suffered a fractured pelvis.

Thomas, who lives in the 6200 block of South King Drive, was charged with reckless driving, driving on a revoked license and operating an uninsured vehicle, all misdemeanors, police said. He was also issued traffic citations for failure to exercise due care for a pedestrian in the roadway, failure to stay in his lane and executing an improper U-turn. He is scheduled to appear in traffic court June 21.

Iowa man dead after three-vehicle crash on I-80

(JOLIET) An Iowa man died after his vehicle veered into oncoming traffic on Interstate 80 Monday afternoon in southwest suburban Joliet, police said.

About 4 p.m., the driver — identified by police as 31-year-old David Zenteno of Des Moines, Iowa — was traveling west on I-80 when his vehicle crossed into oncoming traffic in the eastbound lanes and struck two other vehicles near the Briggs Street exit, Illinois State Police said.

Zenteno’s vehicle flipped over in the collision and he was pronounced dead at the scene, police said.

It was not known Monday night was caused Zenteno to cross the lanes, which were closed for three hours, police said.

 

UPDATE: Joliet man killed in I-80 crash

(JOLIET) A 66-year-old man was killed in a five-vehicle crash on Interstate 80 in Joliet late Sunday morning.

About 11:25 a.m., Michael B. McCoy was behind the wheel of a Chevrolet pickup truck that crossed from the westbound lanes of the highway into oncoming traffic near the Chicago Street exit, according to Illinois State Police and the Will County coroner’s office

The truck hit an RV and four other vehicles, and McCoy, a Joliet resident, was pronounced dead at the scene, authorities said.

Two people from one of the other vehicles were taken to Silver Cross Hospital in New Lenox and were released Sunday afternoon, state police said.

The driver of the pickup was at fault and no citations will be issued, police said.

 

Motorcyclist critically hurt in crash with van in Des Plaines

(DES PLAINES) A 21-year-old motorcyclist was critically hurt when he struck a minivan Wednesday night in northwest suburban Des Plaines.

The motorcyclist was traveling southbound on Mount Prospect Road when he struck a minivan that had partially turned left onto Mount Prospect from Golf Road at 6:53 p.m., according to a statement from Des Plaines police.

The man was thrown from his motorcycle and taken to Lutheran General in Park Ridge, where he is listed in critical condition, police said.

Additional details were not immediately available late Wednesday.

UPDATE: Police: Man killed, 2 injured in Stevenson Expressway crash

(CHICAGO) A man was killed and two others were injured Wednesday morning in a three-vehicle crash on the Stevenson Expressway (I-55) in the South Side Bridgeport neighborhood, authorities said.

Two SUVs and a car collided about 5:45 a.m. in the inbound lanes of I-55 near Throop Street, according to Illinois State Police.

A man, whose exact age was not known, was taken to Mercy Hospital and Medical Center, where he was pronounced dead, according to police and Fire Media Affairs spokesman Joe Roccasalva. The Cook County medical examiners’ office could not immediately confirm the fatality.

Another person injured in the crash was taken in serious-to-critical condition to Stroger Hospital, Roccasalva said. A third person was taken in fair-to-serious condition to Mercy Hospital.

Three northbound lanes of the expressway were closed to traffic, but were reopened by 8:20 a.m., according to police.

Two killed when motorcycle strikes pedestrian in Plainfield

(PLAINFIELD) Two people died early Tuesday when a motorcycle struck a pedestrian in southwest suburban Plainfield.

Sean Fitzpatrick, 27, was riding a motorcycle on Route 59 near Lockport when he apparently lost control and the bike and it struck a pedestrian, according to the Will County coroner’s office.

Fitzpatrick, of Plainfield, was taken to Presence Saint Joseph Medical Center in Joliet, where he was pronounced dead at 12:39 a.m., the coroner’s office said.

The pedestrian was pronounced dead at the scene at 12:55 a.m., but additional details were withheld pending notification of kin, the coroner’s office said.

Autopsies were scheduled for Tuesday, according to the coroner’s office.

Plainfield police did not release details about the crash Tuesday morning.

 

Bicyclist struck and killed by vehicle in Franklin Park

(FRANKLIN PARK) A 51-year-old bicyclist was fatally struck by a vehicle early Saturday in west suburban Franklin Park.

Dagoberto Huizar was struck in the 3000 block of Mannheim Road in Franklin Park, according to the Cook County medical examiner’s office. He was pronounced dead at the scene at 6:07 a.m.

An autopsy performed Sunday found Huizar died of head and neck injuries, and ruled his death an accident.

Franklin Park police could not be reached to comment on Sunday afternoon.

Police searching for driver who crashed stolen SUV in Oak Forest

(OAK FOREST) Police are searching for a driver who ran away after crashing a stolen SUV into a utility pole early Thursday in south suburban Oak Forest.

The vehicle was speeding when it crashed into the utility pole and flipped over about 2:30 a.m. near 155th and Cicero, said Oak Forest Police Sgt. Scott Durano.

The driver ran away after the crash and the vehicle was reported stolen out of Markham, he said.

About 1,600 ComEd customers lost power after the utility pole went down in crash, spokeswoman Liz Keating said. Power was fully restored by about 3:15 a.m.

Durano said officers with the Oak Forest Police Department were searching the area for the driver.

Driver charged in fatal rollover on frozen Lincoln Park lagoon

(CHICAGO) A man has been charged with driving under the influence in connection with a fatal rollover crash that occurred on Lake Shore Drive near the Lincoln Park Zoo early Monday, police said.

Rafael Rodriguez, 42, was charged with aggravated driving under the influence causing an accident or death, misdemeanor driving under the influence, driving without a license, driving on a suspended license as well as a citation for failure to reduce speed and stay in a lane, police announced late Tuesday.

About 1:40 a.m. Monday, Rodriguez was driving south “at a high rate of speed” in the 2200 block of North Lake Shore Drive. He lost control of the vehicle, hit a guard rail and then a tree before the vehicle came to a rest on its roof on top of a frozen lagoon, police said.

A passenger in the vehicle, 37-year-old Claudia Beruben, of the 12400 block of South Laflin in Calumet Park was taken to Northwestern Memorial Hospital, where she was later pronounced dead, according to the Cook County medical examiner’s office.

Rodriguez, of the 3900 block of North Ashland, remained hospitalized at Illinois Masonic Medical Center Tuesday, police said.
